چکیده
A 67-year-old man took labetalol (Trandate) 400 mg thrice daily and clonidine 0 3 mg daily from June 1977 for hypertension. Methyldopa had been withdrawn because of diarrhoea, and propranolol and clonidine had proved ineffective. Twelve weeks after beginning labetalol he developed an itchy eruption on his penis, which spread to the trunk and limbs. When he attended hospital 10 weeks later the eruption consisted of reddish purple macules and papules, many of which were oval and had a pityriasis-rosea-like distribution on the trunk. On the legs many of the lesions were bullous. Wickham's striae were present on the glans penis, and typical lichen planus lesions were also seen on the limbs. Biopsy of one of the lesions showed an infiltrate of mononuclear cells in the upper dermis associated with liquefactive degeneration of the basal layer and early bulla formation. Some necrotic keratinocytes were present in the epidermis (figure). Labetalol and clonidine were replaced by atenolol and Moduretic. Dilute topical steroids were given for symptomatic relief, but within two days after altering his antihypertensive treatment his symptoms had settled and topical agents were discon-

Diabetes mellitus in normal pressure hydrocephalus.
Seventeen of 33 patients (51.5%) with normal pressure hydrocephalus were discovered to have diabetes mellitus. This was significantly greater than the 12.1% incidence found in age-matched control subjects. The diabetes was not accounted for either by the patients' ages nor by their physical inactivity due to hospitalisation. Diabetes mellitus concurrent with normal pressure hydrocephalus may re...
متن کاملComparing the Efficiency of Two Treatment Methods of Hydrocephalus: Shunt Implantation and Endoscopic Third Ventriculostomy
Introduction: Hydrocephalus is one of the most common diseases in children, and its treatment requires brain operation. However, the pathophysiology of the disease is very complicated and still unknown. Methods: Endoscopic Third Ventriculostomy (ETV) and Ventriculoperitoneal Shunt (VPS) implantation are among the common treatments of hydrocephalus. In this study, Cerebrospinal Fluid (CSF) hyd...
متن کاملNormal Pressure Hydrocephalus Presentation with a Large Pseudomeningocele
Background and Importance: This interesting case is about the presence of normal pressure hydrocephalus and a large pseudomeningocele at the same time after 13 months of posterior fossa surgery. Although the occurrence of a pseudomeningocele following posterior fossa surgery is not so rare, such a late large pseudomeningocele development with signs and symptoms of NPH after 13 months of surgery...
متن کاملBilateral Carotid-Cavernous Fistulas: An Uncommon Cause of Pituitary Enlargement and Hypopituitarism
Carotid-cavernous fistulas (CCFs) are rare, pathologic communications of the carotid artery and the venous plexus of the cavernous sinus. They can develop spontaneously in certain at risk individuals or following traumatic head injury. Typical clinical manifestations include headache, proptosis, orbital pain, and diplopia. We report a case of bilateral carotid-cavernous fistulas associated with...
متن کامل'Do no harm': management of craniopharyngioma.
Craniopharyngioma management is challenging. Although histology is benign, the tumour can be clinically aggressive with local invasion and frequent recurrences. Extensive morbidity may be present at diagnosis and furthermore, occurs as a consequence of neurosurgery and radiotherapy. Hypothalamic symptoms can have a devastating effect on quality of life and may reduce life expectancy. This case ...
